I've had one who lied about an operational (hit estop by accident) and she was terminated the same day on Logger's Run at Michigan's Adventure. When that ride estops, people get stranded because water drains, and boats just bottom out. Then it takes about 10-15 mins for it to get working again.

At Cedar Point... my ex-supervisor (from Michigan's Adventure) had a false dispatch and estopped the ride himself (2 operationals in one day), and got stuck on Power Tower. This was the same sup where I got stuck on controls and I witnessed him commit about a dozen operationals in loading when he took over for some of the other host's temporarily while they had to fill out training paperwork. Last I heard, he had gotten promoted at Michigan's Adventure to management.

About the only other time I had to estop was when I had to run a kiddy ride one afternoon, and the kid started crying where the mom insisted on getting the kid. At least I was able to restart it and those on the ride got an extra bit of ride time.

^^ I didn't realize that a flume ride drained when E-stopped. I guess it makes sense though.
I definately feel it's a hard choice no matter what the mistake. One of those things where managers, employee witnesses and anyone else involved need to sit down and talk about what happened and what the circumstances could have been or were. No one ever wants to let anyone go, But when you put someones life in danger you're only asking to be terminated.

I agree with how the consequences are. If you screw up big time in this industry you could cost somebody their life. The consequences have to be harsh in order to the point across to not screw up and to check everything.

I agree with those who said that it depends on how bad the situation is/was. If you lie about something then you should get thrown out automatically.
